Pest Observation Checklist
A photo is more useful with context. Use this sheet to record what you see before choosing a treatment. Keep the completed copy for yourself or share it directly with your technician.

Record the sighting
- Date and time:
- Room or outdoor area:
- One insect, a trail, a nest, or repeated sightings?
- Approximate size and color:
- What changed recently: rain, deliveries, travel, or landscaping?
Make a useful photo record
- Take a wide photo showing the location and a separate close-up.
- Include a size reference beside the pest only when you can do so without handling it.
- Note whether the insect had wings and whether more appeared later.
- Photograph visible signs without opening walls, moving nests, or disturbing suspected termite activity.
Check the surroundings
- Look for accessible food, standing water, leaks, or gaps around doors.
- Record the product name and date of any previous treatment for the technician.
- Note which areas you could not inspect; an unseen area is not a confirmed absence.
- Keep children and pets away from pest activity while arranging identification.
Bring these questions to an inspection
- Which pest is confirmed, and which observations support that identification?
- Is this an isolated problem or repeated activity?
- What work is included, and what follow-up is recommended?
- What preparation or access instructions apply to the proposed treatment?
